Teachers: Thank you for agreeing to provide an evaluation for the student who is applying for a CCCW/Knights of Columbus parish scholarship for the 2026-27 academic year.  Please complete the form below and rate the student on the characteristics listed.  The rating scale is from 1 (Needs Improvement) to 5 (Excellent).  You may also provide additional information regarding your rating or to supplement your rating.  If your rating is either “Needs Improvement” or “Excellent”, please provide additional information as to why the particular rating was selected.
